ARTHROKINETIC THERAPY FOR TFCC INJURY
Anatomy and Function
The TFCC is a complex structure in the wrist, comprising:
- Triangular fibrocartilage
- Radioulnar ligaments
- Ulnar collateral ligament
- Extensor carpi ulnaris tendon sheath
Biomechanical Functions:
- Forearm rotation
- Wrist stability
- Load transmission
Injury Mechanisms:
- Fall onto outstretched hand
- Repetitive strain (golf, tennis)
- Direct blow to wrist
Arthrokinetic Analysis:
- Wrist joint mobility assessment
- Forearm rotation analysis
- Muscle length-tension testing (extensor carpi ulnaris, flexor carpi ulnaris)
- Ligamentous laxity testing
Biomechanical Changes:
- Altered wrist kinematics
- Increased ulnar variance
- Decreased forearm rotation
Arthrokinetic Treatment:
- Joint mobilization (wrist, radioulnar)
- Soft tissue mobilization (extensor carpi ulnaris, flexor carpi ulnaris)
- Muscle energy techniques (post-isometric relaxation, reciprocal inhibition)
- Exercise (wrist strengthening, forearm rotation)
Phases of Treatment:
- Acute (0-2 weeks): Pain management, joint mobilization
- Sub-acute (2-6 weeks): Strengthening, soft tissue mobilization
- Functional (6-12 weeks): Advanced strengthening, sports-specific training
